I am not able to see any submissions

I THINK I HAVE DINE MY FORM CORRECTLY BUT EVERYTIME I GO TO SEE IF I HAVE ANY SUBMISSIONS THERE IS NONE. PLEASE TAKE A LOOK AT MY FORM AND LET ME KNOW WHAT I NEED MORE TO DO. I AM NEW TO ALL THIS.
