Want current Form name to show up on submission

Form used to be called Prints. I

1. Cloned it

2. Revised it

3. Renamed it

On part of submission form distributed

"Clone of Prints" shows up as title instead of

"Prints from the Oakville Historical Society".

How can I get the proper name to show up?

I may be cloning, upgrading and renaming other forms.

I believe you are referring to the title shown in the email notifications or autoresponders.  To update that title, while editing your form, click the settings tab, then the emails tab.  Select the email you want to update, then change the title in the body of the email:

Save your settings and you're all set.
