Can conditional logic be applied to individual answers in a multiple choice question where each choice has a different value?

Yes, you can. You can target a new field, a Single Text Entry field to pass the value.

    https://www.jotform.com/help/268-How-to-Insert-Text-or-Mathematical-Caculation-into-a-field-using-a-Condition

Note that you can also assign calculation values for each option. Just follow the steps here to assign: https://www.jotform.com/help/301-How-to-Assign-Calculation-Value.

Thanks. Is it possible to have someone complete the form but have obtaining the calculated answer require submitting their contact info (name, email)? Or would I need to collect that info prior to them completing the form?

If I understand you correctly, you would like the name and email fields to appear based on the answer. If yes, then I recommend you to use the Show/Hide Field Condition. Here's a user guide: How-to-Show-or-Hide-Fields-Base-on-User-s-Answer.

If you are referring to a different issue, please let us know. We might also need to create a separate thread if it is different from the original issue.

    Thank you.

No. all questions are required to generate a total score, but I want to require them to provide a legitimate email (and name) in order to receive their score. So I want to email their score to them rather than providing it on the form page.

Or alternatively, I have the logic set up to calculate the total into a hidden text filed. I could use the email verification or XVerify widget to capture a legitimate email, but how would I incorporate their total (along with a legend explaining the meaning of their score) into an autoresponder email?

Yes, you can hide the Total score field and include it inside of an Autoresponder email by adding the value of the Total score field. The field will be shown inside of the email template by default.

If you also want to add a different legend for each score, you will have to create multiple Autoresponder emails and customize them with a legend explaining the meaning of the Total score. Inside of this guide, you can find how to edit the email template:

How to Edit the Email Template for Notifications and Autoresponders

Then you will have to use the conditional logic to sent an Autoresponder email based on a Total score.

In trying to set up the different autoresponders, the “if” questions don’t really apply. I need to differentiate based on a range of scores not a single value. Is that possible?

Is this the form you are working on http://www.jotform.us/form/72888444784172

You did not perform a calculation. The Total field that you added to your form is a Text field. You should perform a calculation with the Form Calculation widget.

How to Perform Form Calculation Using a Widget

Then you will be able to use Grater Then and Less Then conditions states to set the conditional Autoresponder emails.

Disregard my previous question I figured out I need to set up multiple conditions equaling the range for each email

OK, I got everything in place (I think), but when I completed the form, the actual score number did not appear on the client email. It did in the notification email. The value should be represented by {yourScore} correct?

Nevermind. I am not sure what happened the first time but a second test came out OK, though it was a different score so different email.

Is it possible to use two form calculators in the same form? I have one set up to calculate the clients “pain” and the I want to add a second to measure their commitment. So each calculation would be separate, and the second score would only be for me, not the client.

I put a second one at the end of my form but when I go to set the logic, the previous calculator logic is already populated

OK, I figured out how to add and use a second form calculator in my form but I don’t see
how to prevent the second (commitment score) total from going to the client. Would I just erase that entry from the table in the client emails?

I changed the color theme of the form but it remains unchanged in the preview and when I embed it. How do I fix this? https://form.jotform.com/72888444784172
