Do you have a scale matrix field like this?

Hi ... I'm wondering if you have a field type or widget similar to the image below.  It's a common type for tools like Survey Monkey, etc. I searched the widget library but couldn't find something similar ... but then, there are a lot of widgets!

A similar element is currently called an Input Table in our form builder.
