I've noticed that my forms have been disabled, I've tried to login but my account says that my login was suspended. I've requested a new password but it didn't work either.

Your account (olguiux) is currently suspended. These forms (PagoReservas and PagoReservas_2) triggered the suspension. I checked the forms and I can see that you are collecting credit card information through the File Upload tool and a field asking for the CVV/CVC code.
We need to delete these forms so that we can reactivate your account.
Let us know if you want us to proceed in deleting these forms. We will wait for your response. Thank you.
