How do I make a form with Reoccuring Payments and a One-Time setup fee with CardConnect Payment Processor?

What's the best way to make a reoccurring payment form where we send the link to a customer, they select the service with reoccurring payments and the information is submitted into CardConnect and sets them up with a billing plan?

I would also like to be able to add a one-time setup fee to the form and allow them to pay it at the same time they are signing up for the service with reoccurring payments. How do I integrate this option as well?

If you guys don't provide this functionality, do you know of another solution that does?

Unfortunately, there's no recurring payment or sell subscription available in the payment options.
