Update format of PDF submission

Bonjour,

  Nous auriosn besoin d'aide, nous avons créer un template pour les formualire a faire complèter pour nos patiens, le problème est qu'une fois le formulaire soumis nous voulons l'avoir en pdf pour l'inclure dans notre système de réservation et nous ne sommes pas capable de l'ajuster correctement.. merci de nous aider
