How do I create a table with static text but allow for user entry in certain columns?

     I'm trying to create a table with static data in most of the columns, but I want some of the columns to allow for customer data entry. See screenshot that I've included for an example of what I'm trying to do. Tried the matrix table but it doesn't quite do what I'm looking for. Also I want to make sure I can adjust the width so it doesn't make my form really wide.

    Use Spreadsheet widget.

    By default, all cells are fillable but you can make them ready only in the widget settings.
