Why is my account suspended?

     I am unable to login and access my form. I got a message saying I had been banned from the site for phishing.  I don't even know what that means.  There is a space on my form for people to write down their credit card information. I saw that you have a purchase order template that allows you to collect this information and process it off-line.  I was unable to convert my current template into that template. Is that the problem? We are not using an online processor like PayPal. The school ( this is a school fundraiser) is processing the credit card orders in-house.

    Please note that there are many users using the Jotform forms for phishing purposes, we understand this may not be the case with you, yet we have to act according to our terms.

    I removed the fields from your form in order to reactivate your account, please note that a payment integration is required in order to ask for payment info.
