My website is not quite ready for prime time but when I embedded my form in it the photo upload was grayed or rather yellowed out and wasn't responding when I tried to upload a photo. This is crucial for my website. without the ability to upload a photo, customers can not go on to the next step to purchase. Need your support with this.

I have only published this site for you to look at. It is not yet ready for prime time. Please fix the upload photo option. Otherwise I can't launch the site. Thank you.

I have made a test (did not complete the payment process, of course, you'll see an incomplete payment into your submissions)
And I was not able to replicate the issue. Can you please tell me which browser you use? is this problem present in all of the browsers? I made the test using Mozilla Firefox, and it worked perfectly
I can see you have embedded your form using iframe , which is good when you have several js libraries
Please give it a try into another browser so we need to narrow the problem

I would also advise you to modify the form's url in the current embedded code of your page,
FROM http://form.jotform.us/form/22016001083131 TO http://www.jotform.us/form/22016001083131

Changed it to the WWW.jotform and it's still not working on Safari - iPad

I created the site in Firefox and that's what I use. When I used it on the iPad this is when the issue occurred - so Safari. Any thoughts? I imagine a lot of folks will be using Safari and so need this to work.

This is a known issue between Jotform and ipad/safari's technologies
