Forms no longer working

Hi, I have three forms on a website. One a Contact Us form and two where visitors can upload information to me. Currently, they are all not working, details below:

Contact Us form - keeps asking for mandatory fields to be filled in when they already are.

The upload forms just come back with a "Site Can't Be Reached" error message when you click Submit.

I have a Free account and am getting close to my maximum number of submissions. If I upgrade will it start working again or do I need to redo the forms?
