When I complete a form (as a user) - including completing various input scales - the rating selection doesn't print, only the text in the notes box. I'm talking about before the form is submitted.
When its submitted - as the recipient - I see the ratings. But the user should also be able to print a copy of the entries they have made.
Am I doing something wrong in the form design?

Actually, this is caused by a bug in Internet Explorer 8. Microsoft was able to correct this in IE9.
To be honest, there's not much we can do on our end to get around this serious bug. What I advise you to do is to insert a notice asking your users (using IE8) to view the form in a different browser.
