    Hello Lindy

We appreciate your words towards our service. Please accept my  apologies for this inconvenience, we have performed a database maintenance to improve our quality of service and this caused that some users that have their forms hosted into www.jotform.us  or   www.jotformpro.com , experienced data replication issues.

I have inspected the embedded code into your page. The form is hosted into

    http://www.jotformpro.com

So please modify the current embedded URL in your page,

    FROM http://form.jotformpro.com/jsform/22498866755978

    TO    http://www.jotformpro.com/form/22498866755978

I did as you asked and republished and then the page was blank

I have switched it back.  Having the old form is better than no form at all!

    PLEASE ADVISE

Can you please tell me if this is the updated form ?  otherwise, you would need to log into your Jotform account using www.jotformpro.com and update your form.

Then on your page source code, please make sure the URL of the form gets modified, from http://form  to http://www

I see another difference in the two links

    FROM http://form.jotformpro.com/jsform/22498866755978

    TO    http://www.jotformpro.com/form/22498866755978

    the first one has /jsform/

    the second one has /form/

    could this be the problem?

We had this replication data during the last 2 days due to a database maintenance, so people performing changes on those days were having problems, if you have done updates lately in any other form , you should do the same process, otherwise, there should not be any problem.
