I don't know why the text of my form doesn't show in the pdf of my submission. Even if I choose "show header and text".
Can you quickly help me?

I cloned your form and the Text fields are showing properly after enabling the Show Headers and Text option on the Submissions page.
Please take note that after enabling that option, you need to click the Properties icon again to save the changes. Here's a screen capture:
Are you still experiencing this problem? If yes, then please try to reset the PDF template using the PDF Designer. Here's how: https://www.jotform.com/help/384-How-to-Customize-PDF-Submissions-Report.
In the PDF Designer, click the "Reset" button to reset the template.

Thanks for your answer. But it still not work. I tried resetting the pdf template and it still don't show texts and headers. Any idea why?

I have just tested this with a form on my end and it seems to be working:

I was able to replicate mentioned issue on your from and on my cloned form. Headings are shown on the submission page and inside of PDF Designer, but not when the PDF reports is downloaded or when the PDF report is Previewed inside of the PDF Designer.
It seems that the issue is related to customizing of the PDF report. When any change is made the headings will be removed (It seems that you removed the image). For now, I can only suggest you to reset the PDF again and to use the PDF report as it is without any edits inside of the PDF Designer.

Ok, so it's a bug? Because I used it for a few submissions and it worked well, then I de-activated it for a while and when I re-activated it it didn't worked like before.
Do you think the bug will be solve soon?

My colleague has already raised this issue to our backend team. As per checking the status of the ticket, one of our developers is already looking at this issue and it is marked as Important. However, we couldn't provide an ETA for when this will be fixed. Rest assured you will be notified in this thread if there are any updates.
