This is a follow up to my previous question. I made the change that Mike suggested, and I am still having problems with the form loading and response time to mouse clicks. Would you be able to look at the form and its conditions and make recommendations on how to streamline it or improve its functionality?

I have just tested your form and it seems to be loading fine on my end, here is a screen-cast about what I can see:
Is the issue still happening on your end?
I would recommend you to test using incognito mode since this might be a cache issue. If the problem persists, please let us know which browser or device did you use, I'm on Google Chrome the latest version now.
