How can I show the witget of map location correctly

Hello I´m trying to put the map location widget on my form but I have 2 problems, I can´t configure the widget to show my bussines address and I can´t show it on the right side of the entries.

When I configured the first time my forms ít worked perfectly, but I wanted to make a change and everything got wrong.

I have 2 forms, one with the address in Caracas (wich I haven´t touched and still works), and the other with the address if Tenerife wich shows the google map on the botton and doe´st show the address correctly. I want de Tenerife form to look like de Caracas form.

If you want to show your address location, that is not the correct widget to use, please try using the "Show Map Location" widget: https://widgets.jotform.com/widget/show_map_location
