My login and form has been autosuspended

I created a basic rental agreement form but it looks like my login and form have been disabled.

Can you pls enable it ? 

I read that this can be due to the anti phishing tool but this is a rental agreement which I took from a template in your website.

Why is it disabled then ?

Pls enable.
