Why QR code is Not working?

Hey, i have tried to follow help guides and instruction from the forums but to no avail.

My QR simply doesn't show as supposed to. Please can anyone educate me as to where i am going wrong?

In your Dynamic QR Code widget, please change the field label to "Name" since it is the correct label of the "Name" field.

I have cloned your form and corrected the label of your QR code, and now showing it is showing in the submissions page.

Banging, such a school boy error. Next question is can the QR hold two bits of information?

I'm not sure which information you want to store in QR code. But, if you meant to store, for example, two fields values in QR code, you can do that by combining two fields in one (with conditions) and then by using the Dynamic QR code widget, you can create a QR code.

For example:

-And with the below condition we copy values from two fields into one:

-Finally, we create QR code from that result field:
