Here is my colleagues reply to that thread.
Your account (mohamedarif) is currently suspended. The form (Application for Business Visa in the Maldives) is the one triggered the suspension. I checked the form and I can see that you have fields that are asking for sensitive information.
Please take note that collecting sensitive information is not allowed here in JotForm. I recommend you check the Terms and Conditions.
We need to delete the guilty fields so that we can reactivate your account. If you really need to collect the Passport information, then you need to have the following in your form:
In regards to the File Upload field where the user can upload a sample copy of the passport, we need to remove this completely.
