Doubt about TEMPLATE

    I created a form and wanted to make another one equal but I do not know the name of the template.

    The form that is embedded on the website you provided has a custom Theme. You can check it inside of the Custom Tab of the Form Designer.

    If you want to create another form that is equal to the original form, you can clone your original form:
