Why the options are constantly changing in multiple choice field?

I have multiple choice questions in my form, they are in order by date. When I preview all looks fine but once I embed into Muse it still looks right, once I publish it the multiple choice options change position, messing up the dates, if I hit refresh it will change the order again. I tired embed and publish in all formats and it still does the same thing. HELP, my client needs to send this out and it can not go out like this. This use to work perfectly but since your site was redesigned its all buggy. What can we do???
