On Oct. 22, we received a "new" application from one of our staff. The original application date was 5/24/16 (when we hired her). Any idea how this happened? She was as surprised as I was.

Whats the form in question, and the submission ID? In order to get the ID, you need to go to your form submissions page: https://www.jotform.com/help/269-How-to-View-Form-Submissions , then, you need to click on the "More settings: gear icon and check the ID box.
Probably, you or her submitted the form in edit mode? Or, is it a new submission?

The submission ID is 3845094783729056233 for Marquila Wiseman. It is a job application. Neither she nor I had touched it since 5/24/16, and suddenly there it was in the inbox on 10/22/17, masquerading as a "new" application.

I have checked the form submissions and noted that a submission was made as shown below:[2017-10-22 15:24:38] Submission #3845094783729056233 Form #11225837414 SENT to email@example.com from Marquilawiseman@gmail.com, via SENDMAIL.

I have checked email logs for your form with the supplied submission id. Logs show that there was a POST request that is coming from KETCH on exact same date as your submission(22/Oct/2017 19:24:38). Submission was made from an Android mobile phone version 5 which uses Chrome version 49.
