Unable to submit embedded form

I have a form on my churches website and it's not sending like it should. There is a send button and everything. Now, once filled out, it won't do anything once the send button is pressed. The page just sits there, PLEASE HELP ME.

I tested your embedded form and I was able to replicate mentioned issue. Your standalone form is working properly.

You embedded your form using the full source code. Did you recently change your form? Please note that you need to re-embed your form when you any edit is made because the source codes that are currently embedded to your page are not automatically updated. 

Please re-embed your for to see if that will resolve your issue. If the issue is not resolved that embed the form with the iFrame code. Inside of this guide you can find how to get the iFrame code of your form:
