500 Internal Server Error

    I see 

    500 Internal Server Error

    How can I do ?

    I've checked the form and was able to display correctly. I believe there is some other problem with your webserver as it unlikely that form script code will display 500 internal server error. I would suggest you to please remove just the script embed code from your webpage and see if webpage is displayed normally. I would also suggest you to please try to embed your form using its iFrame embed code and see if that solves your problem.
