Account was Suspended

I had created several forms at the end of July. I had just received an email saying my form wasn’t found. I tried to login to my account and it said it was suspended. 

HELP!!!! WHY????

As per checking, your account A1439916 was suspended because one of your forms (http://www.jotform.us/form/72121661084146) is collecting credit card information which is against our terms of use. I have removed the guilty fields in your form to activate your account. You can now access your account and forms.
