não estpu conseguindo o acesso

Estou tentando acessar a minha conta, entretanto não estou conseguindo obter acesso, poderia por gentileza liberar o meu acesso ALVESNEILON.

I see that your account has been suspended due to the form http://www.jotformz.com/form/72973977984685 which is having a password field. It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing. 

I have now removed the password field from the form and activated your account. You should be now able to login to your account and manage your forms normally.

Please refrain from adding such fields to the form to avoid any further suspensions on your account.
