I am looking to have a group of checkbox fields underneath each other. For example
Section A Please rate your salesperson on the following:
Knowledge q poor q Fair q Good q Very Good q Excellent
Accessiblility q poor q Fair q Good q Very Good q Excellent
poor q Fair q Good q Very Good q Excellent
Effectiveness q poor q Fair q Good q Very Good q Excellent
Educating q poor q Fair q Good q Very Good q Excellent
I can't seem to find a way to get the catagory and the checkboxes on the same line (with the Poor, fair, good, etc) or to have multiple lines under the Section A heading without makiing each one it's own field with the catagory being the heading of the field (and on a different line). How can I accomplish this?

For the type of fields you are looking for, I think a radio button input type will fit better in your case

That would work well! How do I get this type of table set up?

It's called Matrix under Survey Tools. Check this screen: http://pbrd.co/TA7oR3
