How can I make the fields do what I want?

    I am looking to have a group of checkbox fields underneath each other.  For example

     

    Section A    Please rate your salesperson on the following:

    Knowledge       q poor   Fair   Good   Very Good   q Excellent

    Accessiblility        q poor   q Fair   q Good   q Very Good   q Excellent

    Professionalism   q

    poor   q Fair   q Good   q Very Good   q Excellent

    Effectiveness       q poor q Fair q Good q Very Good q Excellent

    Educating            q poor q Fair q Good q Very Good q Excellent

    etc.

    I can't seem to find a way to get the catagory and the checkboxes on the same line (with the Poor, fair, good, etc)  or to have multiple lines under the Section A heading without makiing each one it's own field with the catagory being the heading of the field (and on a different line). How can I accomplish this?

    For the type of fields you are looking for, I think a radio button input type will fit better in your case
