How to create Form Columns and how many columns I can create?

Let me have the following questions

1.How I align your zodiac sign and your gender side by side?

2.How do I change from square to diamond etc?

3.How many columns do I have, though, the columns of the guide is two?

I did check your form and found that you have already placed the form fields side by side. Please check the screenshot below:

What exactly you want to change. I would suggest you to please explain your requirement a bit more in detail and we will try to help you.

3.How many columns do I have, though, the columns of the guide is two?

You can add a maximum of six columns in your form. Please note that the more the number of columns, the width of form field will be smaller. Please check the screenshot below on how to increase the number of form columns:

That seems funny.

I checked my question again.

I have a still problem. Let me present the gif as follows.

I have tried  it based on your guide, though, the problem came out as follows

That is mine does not have 

1.Enable form columns

2.Number of columns

What is the distinction between them?

You are not able to see the form column option because you have not shrinked your form field. Please note that the form column option is only available if your form fields are shrinked. Please check the screenshot below on how to enable form column in your own form:
