    I did check your form's submission page and I see that you have received several blank submission. I am not sure what has caused this issue. Did you test form by submitting it without any input? None of your form fields are required fields and user can submit your form without any input in form fields. 

    I did test your form by sending test submission and it was captured successfully. Please check the screenshot below:

    I would suggest you to please make few of your form fields required which will ensure that no user is able to submit form without any form input. Please check the screenshot below on how to make your form field required field. Please check the screenshot below:

    I have tested the form with input on china side and it show up blank. File attached is the data should appear.

    I'm sorry for the trouble caused to you. 

    Thank you for sharing the screenshot. Looks like there is some problem but I am not sure what is causing this issue. I checked our email logs and found that submission emails are also blank. Please check the screenshot below:

    Have you embedded your form in any webpage where you are having this issue with? Please share the webpage URL and we will take a look. Accordingly I will also report it to our backend team as well.

    Thank you for your clarification and our sincere apologies for any inconveniences this may have caused.

    I have done further tests by filling the form and submitting using different devices that is i have tested using my laptop and my phone. Below are some of the results of tests done.

    Would you kindly share which phone you are using to fill the form and also what happened when you are using your Laptop to fill the form?

    This will make us understand better what the issue is as I have not experienced the challenge you have experienced.

    Also I would like to request if you could make some fields as required so as o avoid empty submissions.
