How can I recover my jotform account please?

Your account has been suspended because your form with ID 72336640410346 has been flagged for suspicion of phishing by capturing sensitive user information.
The form in question is requesting users to share Drivers License details which is against our terms and conditions.
In order to reactivate your account kindly confirm we can delete the question that is requesting for Drivers License. Once we remove the question then your account will be activated
