This same form that is having issues with the date picker is also NOT showing as disabled even tho as per the screen shot, I have disabled it. Yet, when I enter the url from an outside source, it displays the form. I've had so many issues with these forms that I am at the point of not renewing my account with Jotform. :-(

I opened the page URL (https://form.jotform.com/tvc_wine/tvc-membership) you have provided and I can see that the form is disabled. Here's a screenshot:
Is the issue resolved? If you're referring to a different form, please let us know. Thank you.
