Completed Form Distribution?

Can I create a single web-based questionnaire that is e-mailed (upon completion) to one, or a number of e-mail addresses which are entered into the questionnaire by the person filling in the questionnaire?

You could setup autoresponder emails with links to your questionnaire:

One for each email field in the questionnaire.

So in that scenario David, would I be able to use FORMFIELDS in the recipients tab of the autoresponder?

Meaning, the person filling out the form would enter an e-mail address and I would reference the value of their answer (the e-mail address) using FORMFIELDS which would then dynamically e-mail a copy of the completed form to the intended recipient...
