It stays yellow... and will not allow them to submit their entry questionnaire? Can you look at why this might be?

I did a test submission in your form (please ignore it), but I wasn't able to replicate this issue:
Is your form embedded in a website?
If it is please share the URL so we can check further.

used to have a white background and now...it stays yellow. It is not on a
website...direct from jotform...no URL....but did that app change?

I see that you have been using "Section Collapse" element in your form. May this be what's causing confusion for your patients? Your patients need to click on this section to see the "submit" button. As my colleague mentioned that I cannot replicate the issue either.
If this is the source of the problem, I would like to recommend you to remove the "Section Collapse" element from your form to prevent the confusion.
