When I preview the form, I cant get past the welcome page....nothing happens when I click on start

    I have just checked your form and was able to get it working on my end, here is a screen-cast about it:

    Are you still experiencing the issue? 

    If so, please test your form using incognito mode, if the problem persists, please take a screenshot of the web console and provide us that, we will be glad to have another look on your form.

    Unfortunately, your screenshot did not reach this thread, please try re-uploading it following the guide I shared on previous reply. 

    Please do not forget to include the console report.

    I would also recommend you to try using a different browser since this might be a cache issue.
