I created a new account last Friday, and still am unable to login. It tells me, "Login failed - AUTOSUSPENDED"
I have changed my password twice and have still been unsuccessful.

The reason why you cannot log in is that your account got automatically suspended by our anti-phishing filter tool.
I have now re-activated your account back to good status. You can now use your account and forms properly.
