I am not sure why I am not getting emails about submissions of my form. I added my email address under "setting up email notifications" and when I did a test I received it but when I sent it out to someone to complete and she completed it I did not get an email. She also said there is no submit button on her end of the form. Please assist.

You tested the Email Notification with the 'Test email' button and because of that, you received an email to your email address.
Your actual form does not have a submit button and your user was not able to submit it when the form was filled.
You have to add a submit button to your form. Add it to the bottom of the form inside of the Form Builder.
