Login failed - AUTOSUSPENDED

    I am trying to create a form to allow clients to request their account be reset by a customer service representative however, my account has been AUTOSUSPENDED  Can you please reset my account?

    Upon review of your account it was found form:

    http://www.jotform.us/form/73033461365149 contained the word "password".

    That word triggered the suspension since it is evaluated as request for login credentials which are not permitted by our Terms of Use.

    We have removed the guilty field. Please reword your title as to not to draw unwanted attention.

    Your account was reactivated and your form made available for you to continue developing it.
