Is this possible to do aside from using the conditions? I found trying to do this according to conditions did not work. Is it possible to do through the form's source code? or another way for an experienced programmer?

Good news, we already had that feature. It's called Submission Limits. You can now limit your submissions or disable a form on a specific date/time. Here's a guide for that: http://www.jotform.com/help/137-How-to-Limit-Submissions-in-a-Form

Follow through on this:

This is is supposed to on 10 Sep at 9AM (see picture on preferences) - https://www.dropbox.com/s/0h0hgzs4bucdujc/jotformexpirydate.png
But the form is still visible. Anything else i need to do?

Thanks idarktech. How can I get it to enable & disable at a particular time when that menu only allows the for the date to be changed? The other method I am aware of was placing a date/time field on the form and going into the css and hiding it and setting conditions to show or hide it according to the date. The problem is I was never able to get that to work according to the time. The date would work, but the time never worked

There seems to be a bug, I'll inform our developers about this to go with the further investigation. We'll let you know once we received any informations regarding this. Thanks for the screen, very helpful.
It's only for 'disable a form when specified date/time passes'.
You seem to have few forms, may we know the FORM ID in which this problem occurs so we can test it here? We would be happy to report this to our developers if we'll be able to reproduce the problem. Thank you.

I am not sure if I have it setup yet. I created one awhile ago to test it out and I never got it to work.

Can you try it with the 24 HR format of your date/time field, then change your conditions with 24HR format time. Example:
Set your time field to 24HR:
Use 24HR format in your time condition:
Check this Example Form:
Two two fields (full name and submit button) hides because date/time passes 2012-09-11 13:13
