Why is my account Suspended?

    I changed my password but it's saying my account is suspended. What do I need to do to reactivate it?

    Upon review of your account it was found form:

    http://www.jotform.us/form/63275554637161 is asking for credit card information which is not allowed per our Terms of Use.

    Please refrain from suggesting to leave such information in the forms, for collection of such is not permitted.

    Thank you for this information. Is there a way I can edit the form to
    remove this information or delete the form all together? I'm not able to
    log in and I'd like the chance to remove this and keep my account.
    Please advise?

    You might need to wait 30 min before the system unlocks if you have attempted to login several times. But your account remain active to recognize you.
