I cloned your form to test it and was unable to reproduce the issue:
When I tested your copy of the form, the layout in the preview before submit was the same.
Let us know if the header image is still off to the right in when viewing it on your end and we will take another look. It does appear to be correct at this time.

Hi David, this issue now seems to be resolved, thank you. I am also trying to add an image to the top of my submission PDF, but I think that I have the image name wrong. This is how I am entering it the image name after pressing the "insert image" button in the "Customize PDF" view:

Thanks. I added the photo and it worked, but when I save the submission PDF and then reopen it, the image is no longer there. Here is the way that I saved the form:

Also, in the PDF that is attached to the actual submission/confirmation email, the header image does not stretch across the entire PDF for some reason. Do I need to create an image with a wider width, or can something be changed so that the image stretches across the width of the form?
Also in the actual submission PDF that was attached to the email I received after submitting a test form, the field name to the left of the large matrix in this image (on the second page of the PDF) keeps resizing itself after each time I manually resize it so that the responses related to the bottom three fields (Additional Material 1, Additional Material 2, Waste Diversion Rate) get pushed down and the formatting gets altered (image below).
Please advise on whether there is a way to fix these issues. I have only had luck with the formatting not being altered and the PDF staying the way that I last saved it when I don't add any images or extra text fields. Thanks for your help.

As long as the image is being included in the PDF, what is being shown in the preview should not matter. As for the sizing, make sure the image itself is a bit wider.
When you add images and resize fields, it can throw off the layout of the PDF. For your case, I would add the image to the form itself, then include headers and text from the form in your PDF's:
Use this option with the default PDF layout.
