Account Suspended

I just realised my account has been suspended and forms on my site(s) are disabled and i have no clue why it's suspended

Please advise

to be collecting ssn, driver's license and banking information. This is illegal, for it is considered a phishing activity.

If you do have to collect this type of information you need to add a signature field.
