When I get the submission emails from members they have the wrong wording in the subject line. Can you please tell me how to change that each month?

You should be able to modify the form's email autoresponder subject line by going to your form's settings, selecting the email alert, and then updating the subject line there. This guide will tell you more the details on the process: https://www.jotform.com/help/26-Setting-up-an-Autoresponder-Email
