I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.

You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm including SSL and Encrypted Forms features.

Please refer to the thread link below that can help you with more information.

I have now activated your account allowing you to make necessary changes to the form. If you do not want to use the encrypted forms feature, it is required to remove the guilty fields on the form to avoid any further suspensions on your account.

    Thanks!
