How can I convert a form to the old design

    Second, I originally created the form using my own Jotform account, then I put the form up to share with everyone as I wanted to pull out and use the form for my nonprofit client.  I found the form and used it for the purpose but the form now has a different design than the one I created. How can I revert it back to appear like the original form style/design?

    For the form layout, while editing your form, click the Settings tab.  From there, click "Show More", then scroll down a bit until you see "Form Layout":

    Change this to "All Question in One Page" to convert the form to the old layout.
