I have added a screen shot as i am stuck at this point. The instructions say to include the recipient email from the drop down but i am just getting the dates i have added to the form.

I checked your form and I can see that there are no email fields present. Please insert an Email field into the form.
After that, go to the Autoresponder settings and choose the "Email" field in the "Recipient Email" dropdown.
