How do you create the questions on one page?

Every time I add an element it creates a next button and multiple pages. How do I create multiple elements on one page? 

The layout of your form is 'Single Question in One Page'. If you want to create multiple elements on one page, you have to change the layout of your form to 'All Questions in One Page'. This can be done on the Form Settings page.
