The quiz is used for engagement purposes. To keep visitors on our site and also to sign up for our services.

You are to pick a characteristic that best describes you from 3 multiple choices.

Choice A signifies a model characteristic. Choice B signifies a dancer characteristic. Choice C signifies both a model and dancer. The value attached to choosing choice A=2 | B=3 | C=4. There are only 5 questions.

After completion of the quiz and it's been submitted. I want the summary to show the total number chosen for each response and say which career field you are more inclined to be successful at.

I need assistance with the conditions for Total Score. Please advise.

I checked your form http://www.jotform.us/form/73054174284153 and you started well. You assigned calculation values to your options in each radio field and that is a correct way to do it.

But to calculate the total score, you will not use the conditional logic, you will just sum all fields in Form calculation widget:

    1509958623screencast Screenshot 10

That way, whatever is chosen in options will calculate and sum in the Total score.

Regarding each choice sum, the only way I see that working is to add additional number field for each option:

    1509959528Screen Shot 2017 11 06 at 10 Screenshot 21

Those additional fields should be hidden, and then you input values in them by using these conditions:

    1509959564Screen Shot 2017 11 06 at 10 Screenshot 32

And you do that for each block.

Then you add three more form calculations widgets that will sum all number1, number2, and number3 fields individually.

    Hope it helps.

Let us know if you have any further questions.

    Thank you!

Here is how to add number fields:

    1510035917screencast Screenshot 20

And you do that for each radio button block.

Then you can create conditions like this:

How do I get the total finish to show each chosen response calculated total and respond with the appropriate phrase? Image 1 Screenshot 31

And you do that set of conditions for each block as well.

And finally you can sum 1stNumber1 +  2ndNumber1 + 3rdNumber1 + 4thNumber1 + 5thNumber1 and that's your number of choices for A. You can add them to sum with Form calculations Widget:

    1510036206Screen Shot 2017 11 07 at 7 Screenshot 42

Similarly, you do for Number2, and Number3.

If you need any further assistance please let us know.

    Thank you!

You are able to receive the results of the submission on your email upon submitting the form because the Notification Email you have set.

If you want the result or total to appear on your form, you need to unhide the Total fields.

Upon checking the calculation you have set on your Total field, here's what it looks like. And it is calculating fine on my end.

If this is not what you are expecting for the total calculation, can you please tell us more details about what you want to achieve on your form?

Let us know if you need further assistance.

    Hey,
I need to know how can I display the results after form submission rather
than sending the results via email. Is there a way?
I tried editing the autoresponder, etc, but it does not work. Please
    advise.

You could show all results right before the submit, by using the Text field, for example:

    1510131948screencast Screenshot 10

So it will show like this:

    1510132020Screen Shot 2017 11 08 at 10 Screenshot 21

    Could that work for you?

    Please let us know.

    Thank you!

Is there a way I can just have the result to display the highest choice
rather than show all three totals?

    1. I need my survey to display the choice (You are a model, dancer, or
both) at the end of the survey. I'm running into the problem when there is
different variation of choices it does not display 'You are a model', etc
instead it reads 'Thank you for your Submission'. I'm not sure if my
calculations are off or I need to integrate an additional widget. Please
    advise.
    2. When I embed the form to my website it's so big. Is there a code to
make the survey small on my website.

1)"Is there a way I can just have the result to display the highest choice rather than show all three totals?"

Yes there is, add the fields into new Form Calculation field, and use the "nth()" math function: https://www.jotform.com/help/267-Form-Calculation-Math-Function-Reference

    2) On regards of your first question of your latest reply, when you say "I'm running into the problem when there is

different variation of choices it does not display 'You are a model', etc

instead it reads 'Thank you for your Submission'.", does it mean that you want to display the results also in the "Thank You" page?

    3) Your last question "When I embed the form to my website it's so big", will be addressed in a new thread: https://www.jotform.com/answers/1294180
