Your form is working fine in my end upon checking and I was able to submit successfully. Please take a look your submission. Can you please try to delete your broswer cache and cookies?

Hi, thanks for aswering. Before deleting my cookies, I tried to access the form using 2 another computers, and the error message appeared again both of them.
I received the test email sent by you, this part works great. But the form still does not appear on my website. I dont think it is a cookie problem! :/ Thank you

I used latest browser of Chrome, Firefox and IE which your form is working fine. Can you please try to open/edit your form on your form builder and save it again?
