    I cannot access my JotForm account. When I enter the Username and password, the word Autosuspend comes up on the input box. 

    I'm trying to complete a form to share with my team to show them how the JotForm system can help them in their fund raising.

    Hi Ron,

    Your account got automatically suspended by our anti-phishing filter tool because it is asking for driver's license which violates our terms of use. Collecting sensitive information is not allowed here in JotForm.

    If you really need to collect this information from the user, then you need to add the following into the form to avoid getting suspended again:

    - An agreement stating the usage of sensitive information. You may use the Short Scrollable Terms widget or Terms & Conditions widget.

    - Digital signature field. You can use the E-Signature widget.
