How can I have the logo that was added to the form to show when I open the form?

I inserted a logo to the form and when I open it on My Forms it doesn't show up.  What can I do so that it is part of the form.

I checked the form (http://www.jotform.us/form/70707642646157) you provided and I can see the logo you added to your form. Here's a screenshot:

I can see it on the preview mode but not the actual form on My forms.

I checked your form and I was able to see the problem.

The logo is not visible when you're in the Form Builder.

The issue is related to Theme applied to the form so, please follow these steps:

1) First backup your Custom CSS code:

So just copy and paste it somewhere else, I'll paste it here as well:

.jotform-form {

    padding : 0px 0 !important;

}

input#input_4_2_1,input#input_4_2_2,

input#input_4_2_3,input#input_4_2_4 {

    pointer-events : none;

    display : none;

}

2) Go to Themes and remove the theme:

3) Then put back the CSS code that we pasted above.

4) Put back the logo:

5) Then you can adjust the width of the form, font size, font etc:

Thank you Nik_C, that really worked.  Customer Service like this makes Jotform much more enjoyable.  

Out of curiosity how can we get the URL for the logo.  It varies depending on the form.

The easiest way is: click the image (logo) and then right-click - copy image address:

Then paste the link where you need it.
